1. PROMOTION DESCRIPTION: The RevDoc, LLC Giveaway ("Giveaway") begins on September 16, 2024 at 12:00 a.m. EST and ends on October 27, 2024 at 11:59 p.m. EST (the "Promotion Period").

The sponsor of this Giveaway is RevDoc, LLC ("Sponsor"). By participating in the Giveaway, each Entrant unconditionally accepts and agrees to comply with and abide by these Official Rules and the decisions of Sponsor, which shall be final and binding in all respects. Sponsor is responsible for the collection, submission or processing of Entries and the overall administration of the Giveaway. Entrants should look solely to Sponsor with any questions, comments or problems related to the Giveaway. Sponsor may be reached by email at giveaways@revdoc.com during the Promotion Period.

2. ELIGIBILITY: The Giveaway is open to legal residents of Florida who are eighteen (18) years of age or older at the time of entry (the "Entrant"). This Giveaway is subject to all applicable federal, state and local laws and regulations and is void where prohibited or restricted by law. Sponsor, and their respective parents, subsidiaries, affiliates, distributors, retailers, sales representatives, advertising and promotion agencies and each of their respective officers, directors and employees (the "Promotion Entities"), are ineligible to enter the Giveaway or win a prize. Household Members and Immediate Family Members of such individuals are also not eligible to enter or win. "Household Members" shall mean those people who share the same residence at least three months a year. "Immediate Family Members" shall mean parents, step-parents, legal guardians, children, step-children, siblings, step-siblings, or spouses.

3. SPONSORSHIP: The sponsor is RevDoc, LLC (also “Sponsor”), located at P.O. Box 18131 Tampa, FL 33679. Email: giveaways@revdoc.com. Sponsor will conduct Giveaway substantially as described in these Official Rules.

4. PRIZES: Only Entrants selected by the Random Drawing (the “Winner(s)”) will receive prize(s). The actual/appraised prize value may differ at the time the prize is awarded. Only one prize per Entrant, per household per drawing will be awarded. Prizes cannot be transferred, redeemed for cash or substituted by Winner. Sponsor reserves the right in its sole and absolute discretion to award a substitute prize of equal or greater value if a prize described in these Official Rules is unavailable or cannot be awarded, in whole or in part, for any reason. The approximate retail value (the “ARV”) of the prize represents Sponsor's good faith determination. That determination is final and binding and cannot be appealed. If the actual value of the prize turns out to be less than the stated ARV, the difference will not be awarded in cash. Sponsor makes no representation or warranty concerning the appearance, safety or performance of any prize awarded. Restrictions, conditions, and limitations may apply. Sponsor will not replace any lost or stolen prize items.

Prize(s) will only be awarded and/or delivered at or to the location specified in the Official Rules (the “Prize Pickup Location”). A Winner, upon acceptance of the prize, is solely responsible for all expenses related to the prize, including without limitation any and all local, state, and federal taxes. A Winner shall not transfer assignment of the prize to others nor shall the Winner request the cash equivalent or prize substitution. By accepting the prize, the selected Winner grants permission for the Sponsor to use the Winner’s likeness, entry information, and name for purposes of advertising and trade without further compensation unless prohibited by law.

Failure to comply with the Official Rules will result in forfeiture of the prize.

5. HOW TO ENTER: An Entrant will be automatically entered into the Giveaway by purchasing a Rev+ Membership during the Promotion Period (“Purchase Entry”) and checking the "I agree to the Giveaway Terms & Conditions" box within the RevDoc App or sending a self-addressed postcard to the following address (“Mail-in Entry”) together with Purchase Entry collectively “Eligible Entry(ies)”):

All mailed entries must be received by the Sponsor at the above listed address during the Promotion Period to be eligible for entry into the Giveaway. All mail in entries received after the Promotion Period will not be qualified as an Entrant.

Only one entry per Entrant will be accepted during the Promotion Period regardless of how the entry was submitted. In the event an Entrant enters the Giveaway by way of Purchase Entry and Mail-in Entry, only one entry will be counted. Each Entrant's entry will count as a single Chance to Win.

While an Entrant can only submit one entry per Promotion Period, an Entrant can earn additional chances to win (“Additional Chances to Win”) by one or more of the following:

1. An Entrant that invites an individual to register on the RevDoc app with the Entrants Invite Code during the Promotion Period will earn two (2) Additional Chance to Win.

2. If the Invited Individual becomes a Rev+ or Affiliate Member with the Entrants' Invite code during the Promotion Period, the Entrant will earn one (1) Additional Chance to Win. However, the Entrant and the Invited Individual becoming a Rev+ or Affiliate Member cannot be the same person and any Entrant that also becomes a Rev+ or Affiliate Member with an Invitation Code during the Promotion Period will not earn an Additional Chance to Win.

3. An Entrant that follows RevDoc on Instagram will earn one (1) Additional Chance to Win.

4. An Entrant that likes our Giveaway post on Instagram, tags 3 friends and shares our Giveaway post to their story will earn three (3) Additional Chances to Win.

Entrant must fully meet all Giveaway requirements, as specified in the Official Rules, in order to be eligible to win a prize. Incomplete entries or those that do not adhere to the Official Rules or specifications will be disqualified at the Sponsor's sole discretion. During each of the Giveaway’s drawings, you may submit one entry, and one entry only.

Automated or robotic Entries submitted by individuals or organizations will be disqualified. Internet entry must be made by the Entrant. Any attempt by Entrant to obtain more than the stated number of Entries by using multiple/different email addresses, identities, registrations, logins or any other methods, including, but not limited to, commercial contest/Giveaway subscription notification and/or entering services, will void Entrant's Entries and that Entrant may be disqualified. Final eligibility for the award of any prize is subject to eligibility verification as set forth below. All Entries must be posted by the end of the Promotion Period in order to participate. Sponsor's database clock will be the official timekeeper for this Giveaway.

By entering a submission, you grant permission to the Sponsor to use, reuse, copy, publish, display, exhibit, reproduce, and distribute the materials in any educational or promotional materials or other forms of media (“Materials”) without notifying you. Entrants agree to be identified by name in any material and participate on a voluntary basis and understand they will not be compensated in any way for the Materials.

6. LIMITATION ON ENTRIES: LIMIT ONE ENTRY PER PERSON PER MEMBERSHIP OR MAIL IN PER GIVEAWAY. No third party entry or entry through any Giveaway service is permitted. Any person who attempts or otherwise encourages the entry of multiple of false contact information under multiple identities or uses any device or artifice to enter or encourage multiple or false entries, as determined by Sponsor, will be disqualified. Use of robotic entry devices is strictly prohibited.

7. AGREEMENT TO RULES: By entering Giveaway, the Entrant (“You”) agrees to abide by the Sponsor's Official Rules and decisions, which are fully and unconditionally binding in all respects. The Sponsor reserves the right to refuse, withdraw, or disqualify any entry at any time at the Sponsor’s sole discretion. By entering Giveaway, You represent and warrant that You are eligible to participate based on eligibility requirements explained in the Official Rules. You also agree to accept the decisions of the Sponsor as final and binding as it relates to the content of Giveaway.

8. ODDS: The total number of Eligible Entries received and Additional Chances to Win earned determines the odds of winning. (e.g. One (1) out of all Eligible Entries and Additional Chances to Win)

9. WINNER SELECTION: The Winner of the Giveaway will be selected in a random drawing from among all Eligible Entries and Additional Chances to Win received throughout the Promotion Period. The Winner will be selected by random drawing (“Random Drawing”).

Random Drawing for Prize #1 will take place on October 28, 2024 following the Promotion Period.

The Random Drawing will be conducted by Sponsor or its designated representatives, whose decisions are final.

10. WINNER NOTIFICATION: Winner will be notified by email at the email address provided in the Entry Information on October 30, 2024 after a Random Drawing. A Winner must accept a prize by email as directed by Sponsor within 5 business days of notification. Sponsor is not responsible for any delay or failure to receive notification for any reason, including inactive email account(s), technical difficulties associated therewith, or Winner’s failure to adequately monitor any email account. The Sponsor is not responsible for nor shall have liability for Winner’s failure to receive notices due to email security settings that may cause notifications to be marked as spam or junk email. Nor shall Sponsor be liable for the Winner’s provision of incorrect or otherwise non-functioning contact information.

If a Winner 1) fails to claim the prize within 5 business days from the time the award notification was sent, 2) is found ineligible, or 3) does not complete and return an executed declaration and release within the specified timeframe, the prize may be forfeited and an alternate Winner may be selected. A Winner may be required to sign and return an affidavit of eligibility and release of liability, and a Publicity Release (collectively "the Prize Claim Documents"). No substitution or transfer of a prize is permitted except by Sponsor.

11. PRIZE PICKUP LOCATION: A Winner must pick up their Prize at 3000 Bayport Drive, Suite 880 Tampa FL 33607. A Winner may, at their own risk, request a third party (“Winner’s Third Party”) to pick up their Prize from the Prize Pickup Location for the purpose of delivery. Sponsor may require additional documentation or affidavits be executed prior to the release of a Prize to a Winner’s Third Party. As a condition of winning the prizes, the Winner must complete, sign and return an affidavit of eligibility and liability and publicity release, where permitted by law, and any other applicable forms required by a taxing authority within five (5) days of receipt of written prize notification or prizes will be forfeited and an alternate winner selected.

Receipt of the prize (offered in the Giveaway) by the Winner is upon the condition of compliance with any and all federal, state, and local laws and regulations. IF THE WINNER VIOLATES ANY OF THESE OFFICIAL RULES, THE WINNER (AT THE SPONSOR’S SOLE DISCRETION) WILL BE DISQUALIFIED, AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED.

12. RIGHTS GRANTED BY YOU: By submitting an Eligible Entry (e.g., text, video, photo, etc.) into the Sponsor’s Giveaway, You understand and agree that the Sponsor, any individual acting on the Sponsor’s behalf, and the licensees successors, and assigns of the Sponsor shall, where permitted by law, have the right to print, publish, broadcast, distribute and use in any media known now or hereafter developed, in perpetuity, worldwide, and without limitation, your submission, name, photo, portrait, voice, likeness, image, statements about the Sponsor’s Giveaway, and your biographical information for news, publicity, advertising, promotional purposes, trade, information, and public relations without any further notice, review, consent, compensation, or remuneration.

13. TERMS & CONDITIONS: In its sole discretion, the Sponsor reserves the right to modify, suspend, cancel, or terminate any and all Giveaway should non-authorized human intervention, a bug or virus, fraud, or other causes beyond the Sponsor’s control, impact or corrupt the security, fairness, proper conduct, or administration of Giveaway. The Sponsor, in the event of any of the above issues, may determine a Winner based on all Eligible Entries and Additional Chances to Win received prior to and/or after (if appropriate) the action taken by the Sponsor. Individuals who tamper with or attempt to tamper with the operation or entry process of the Giveaway or website or violate the Terms & Conditions or any other Official Rules will be disqualified by the Sponsor in its sole discretion. In its sole discretion, the Sponsor has the right to maintain the integrity of Giveaway, to void Eligible Entries for any reason, including, but not limited to: Multiple entries from the same user; multiple entries from the same computer beyond the number allowed by the Giveaway rules; or the use of bots, macros, scripts, or other technical means for entering. Attempts by any Entrant to deliberately damage any website or undermine the legitimate operation of any or all Giveaway may be a violation of criminal and civil laws. If any such attempt is made, the Sponsor reserves the right to seek damages to the fullest extent permitted by law.

14. LIMITATION OF LIABILITY: Sponsor assumes no responsibility or liability for (a) any incorrect or inaccurate entry information, or for any faulty or failed electronic data transmissions; (b) any unauthorized access to, or theft, destruction or alteration of entries at any point in the operation of this Giveaway; (c) any technical malfunction, failure, error, omission, interruption, deletion, defect, delay in operation or communications line failure, regardless of cause, with regard to any equipment, systems, networks, lines, satellites, servers, camera, computers or providers utilized in any aspect of the operation of the Giveaway; (d) inaccessibility or unavailability of any network or wireless service, the Internet or website or any combination thereof; (e) suspended or discontinued Internet, wireless or landline phone service; or (f) any injury or damage to participant's or to any other person’s computer or mobile device which may be related to or resulting from any attempt to participate in the Giveaway or download of any materials in the Giveaway.

If, for any reason, the Giveaway is not capable of running as planned for reasons which may include without limitation, infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, act of God, or any other causes which may corrupt or affect the administration, security, fairness, integrity or proper conduct of this Giveaway, the Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Giveaway in whole or in part. In such event, Sponsor shall immediately suspend all drawings and prize awards, and Sponsor reserves the right to award any remaining prizes (up to the total ARV as set forth in these Official Rules) in a manner deemed fair and equitable by Sponsor. Sponsor and Entrants shall not have any further liability to any participant in connection with the Giveaway.

15. DISPUTES: THE SPONSOR’S GIVEAWAY IS GOVERNED BY THE LAWS OF THE UNITED STATES OF AMERICA AND THE STATE OF FLORIDA, WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. Under no circumstances in any such dispute shall the participant be permitted to obtain awards for, and hereby waives all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than participant’s actual out-of-pocket expenses (i.e. costs associated with entering Sponsor’s Giveaway). The participant waives all rights to have damages multiplied or increased.

16. PRIVACY: Any personal information supplied by you will be subject to the privacy policy of the Sponsor posted within the RevDoc app or www.revdoc.com. By entering the Giveaway, you grant Sponsor permission to share your email address and any other personally identifiable information with the other Giveaway Entities for the purpose of administration and prize fulfillment, including use in a publicly available Winners’ list.

17. WINNER LIST/OFFICIAL RULES: To obtain a copy of the Winner List or a copy of these Official Rules, send your request along with a stamped, self-addressed envelope to RevDoc, LLC, P.O. Box 18131 Tampa FL 33679 attn: Giveaway Dept. Requests for the names of the winners must be received no later than one month after the Promotion Period has ended.

18. SOCIAL MEDIA CHANNEL-SPECIFIC RULES: The Sponsor’s Giveaway hosted by RevDoc, LLC is in no way sponsored, endorsed, administered by, or associated with Facebook, Instagram, X, YouTube, or any other social media channel.

19. By participating in the Sponsor’s Giveaway, You, the Entrant, have affirmatively reviewed, accepted, and agreed to all of the Official Rules and Terms & Conditions.
